6. CEREMONIAL ITEM: MDC Beacon Award. Presentation of the Mayor's Disability Council Beacon Award to Benjamin Amyes, Emergency Response Coordinator for the CCSF Human Services Agency; and Robert Stengel, Planner with the CCSF Department of Emergency Management. The MDC Beacon Award honors the efforts of individuals who exhibit outstanding leadership and who are guiding lights in our community.
This award honors Mr. Amyes and Mr. Stengel for their longstanding commitment to the full inclusion of people with disabilities in San Francisco's Mass Care & Shelter response plan following a disaster. Their innovative work in drafting emergency plans and forging community partnerships has made the City of San Francisco a leader and national model for ensuring disability access in disaster planning.

8. CEREMONIAL ITEM: Remarks by Mayor Edwin M. Lee to the Mayor’s Disability Council on the occasion of the Twenty-First Anniversary of the Americans with Disabilities Act.

9. ANNOUNCEMENT: The San Francisco Public Library and The Independent Living Resource Center of San Francisco present “Lives Explored: Writers with Disabilities – A program in honor of the 21st Anniversary of the ADA.”
Please join us for an evening celebrating the rich literary talent in San Francisco’s Disability Community at the San Francisco Public Library, Main Branch. Reception 5:00 – 6:00 in the Latino/Hispanic Community Room. Reading 6:00 - 7:30 in the Koret Auditorium. Presentation by Marti Goddard ADA Coordinator, SFPL.

Accessible Meeting Information
City Hall is accessible to persons using wheelchairs and other disabilities. The Polk Street/ Carlton B. Goodlett entrance is accessible via a ramp and a wheelchair lift. The other three entrances are accessible via ramps. Assistive listening devices are available and the meeting itself will be real-time captioned and sign language interpreted. The nearest accessible BART station is Civic Center Plaza at the intersection of Market, Grove, and Hyde Streets. MUNI bus lines serving the area are the 47 Van Ness, 9 San Bruno, and the 6, 7, 71 Haight/ Noriega. Accessible curbside parking is available on 1 Dr. Carlton B. Goodlett Place and Grove Street.
In order to assist the City in accommodating persons with severe allergies, environmental illness, multiple chemical sensitivity or related disabilities, please refrain from wearing scented products to MDC Meetings or when visiting the MOD Office.
