Virtual Warehouse Authorization

To become an authorized department official, FAX a completed Virtual Warehouse Authorization  form to the Virtual Warehouse Associate.  Department Heads may delegate authority by signing the submitted form.  Once authorized, a person may delegate the same authority to others within their department.
