Volunteers Needed for H1N1 Clinic Event

Volunteers wanted to help at City clinic sites that are distributing the H1N1 flu vaccine. 

 

Please come to the Department of Human Resources Volunteer Center at One South Van Ness—2nd floor Atrium on Tuesday, October 27, 2009 during the hours of 12 noon to 8 p.m. to register for this volunteer opportunity.  To register as a volunteer in the H1N1 clinics you must:

 

•             Be at least 18 years old.
•             Be a US citizen.  (Required by California government code section 3102.)
•             Bring a valid government-issued photo identification card.
•             Provide a social security number.
•             Take a loyalty oath in support of the United States of America.
•             Be fingerprinted at registration.
•             Complete a conviction history form.
•             Allow a picture to be taken for a Disaster Service Worker ID card
