 Program participants have their own customer service center.  They can call (415) 615-4555 or visit their website at http://www.healthysanfrancisco.org/participants/ for all matters including:

  • Questions about service
  • Haven't received the participant ID card
  • Changes to their participant information

  • And everything else...

The PARTICIPANT CUSTOMER SERVICE CENTER hours are Monday through Friday, 8:30am to 5:30pm.

Healthy San Francisco Medical Home Services (help and information for doctors and clinic staff) is available Monday through Friday, 8:30am to 5:30pm and may be reached at (415)615-4545 (Medical Home Line).

Eligibility Staff - "Certified Application Assistors (CAAs)"/Eligibility Workers who enroll San Franciscans into the program

If you are a Healthy San Francisco eligibility worker or Certified Application Assistor and require assistance, you should first speak with your immediate supervisor. Otherwise, email your question to CommunityOutreach@healthysanfrancisco.org and a representative will respond (usually within 24 hours).

Am I eligible for Healthy San Francisco?

You may be eligible for Healthy San Francisco if you. . .

  • Live in San Francisco and provide proof of San Francisco residency.

 

  • Have been without employer-based or individually-purchased health insurance for at least 90 days from the date of submitting an application, or have lost employer-based health care coverage within 90 days of date of submitting an application due to a change in employment, or who have lost COBRA coverage within 90 days of submitting an application;

  • Are NOT ELIGIBLE for California and/or federally-funded health insurance or assistance programs, such as Medi-Cal.

  • Between the ages of 18 and 64

Different segments of the population are eligible to enroll in the program at different times.

Neither employment status, immigration status nor the existence of pre-existing health conditions are used to determine eligibility in Healthy San Francisco.

 

How do I prove San Francisco residency?

  • Pay stub from a paycheck received within the last 45 days that includes a current address
  • Active school ID card with a current address
  • Unexpired driver's license or state-issued ID card
  • Phone, PG&E, or medical bill received within the last 45 days
  • Bank statement received within the last 45 days
  • Tax return signed within the last 45 days
  • Unexpired matricula (an ID issued by the Mexican Consulate)
  • Rental receipt received within the last 45 days

I meet the eligibility requirements, WHEN can I enroll?

- All SF Department of Public Health Clinic members UNDER the 100% FPL (Federal Poverty Level) who currently have an appointment with their clinic can enroll -- click here to see list of clinics

- All San Francisco Community Clinic Consortium Clinics members UNDER the 100% FPL (Federal Poverty Level) and who currently have an appointment with their clinic can enroll -- click here to see list of clinics

  1. If it is determined that they are with a DPH Clinic (see list of clinics) and meet the above criteria they should the Enrollment Eligibility Unit at (415) 206-7800.

  2. If the caller is part of a Consortium Clinic they should then call the medical home/clinic (see list of clinics) to make an APPOINTMENT to meet with an eligibility specialist!

How do I know if I'm under the Federal Poverty Level (FPL)?

FPL is roughly $10-11K for an individual -- Click here to FPL guidelines

Use the FPL calculator to get a general idea of where you are on the Federal Poverty Level (FPL) and how that relates to the sliding scale payment fees: http://www.healthysanfrancisco.org/visitors/Participant_Costs.aspx

Only way to truly find out is to:
1. Call Center/Medical Home of your choice
2. Make an appointment with the eligibility worker or CAA.

 

How much does it cost?

The participants pay:
  • Participant fee to maintain enrollment in program paid four times per year. But if under 100% of FPL there is no fee.

  • Point-of-service fees when accessing services (insurance equivalent would be co-pay)

The actual fees are determined on a sliding scale. Exact fees will not be known until the participant enrolls into the program.

Participants will be responsible for any cost of care delivered outside the HSF provider network (within and outside of San Francisco)

Can I get access to care outside of San Francicso?

NO, you can ONLY get access to health care through the HSF provider network IN San Francisco.
