Annual Reporting Form

The deadline for returning the 2008 Annual Reporting Form (ARF) was April 30, 2009.  There is no process to grant extensions.  Employers were made aware of this reporting requirement last year, when the initial ARFs were mailed. The 2008 ARF was mailed at the beginning of March; however, blank forms were available online from our website in January.  The OLSE has not determined the penalty for late filings; thus, employers are advised to return late ARFs as soon as possible, without sacrificing accuracy of data reported.

 

The link below will take you to a a fillable PDF, which allows you to type your answers, rather than handwriting them.  You will not be able to save your form or submit it online, so please be sure to print a copy of your form and mail it to the post office box listed at the top of the form.

Employers are encouarged to review the Annual Reporting Form Overview (PDF) and the Instructions for Annual Reporting Form (PDF) to ensure compliance with this reporting requirement.  When completing the Annual Reporting Form (ARF), please keep the following in mind:

  • Only one ARF is required.   If you download, complete, and return the ARF from our website, do not return the ARF mailed to you. 
  • If you use the ARF from our website, you must include your Business Account/Certificate Number, Ownership Name, and Mailing Address in the top, left-hand corner of the page. Failure to do so may result in penalties and other corrective action, as the ARF cannot be processed without this information.
