What does the Disaster Service Worker Card do? The Disaster Service Worker card helps to distinguish City employees from the general public and allow appropriate personnel access to potentially hazardous locations. Access levels are meant to keep employees safe and allow only critical response staff into a hazardous area. The individual assignment of an access level is not an indication of a worker's standing or importance to an event. Each level allows access to hazardous, potentially unsafe or secure zones as determined by Public Safety Officials or the Operations Branch of the Emergency Operations Center.
What does the color coding and numbering at the bottom of the DSW ID card designate? All DSW ID cards have a color code and number. These codes designate who is allowed to enter into dangerous scenes such as collapsed buildings, contaminated sites or other incident. Generally all CCSF employees have a green color code and the number 3. This access level denotes that employees have access to safe and secured areas. Yellow (2) and Red (1) access levels are reserved for restricted, unsecured, and hazardous areas designated for First Responders and Law Enforcement personnel only. If you have a legitimate need for a change in your badge code, you must submit a change request to the UASI Steering Committee. You may obtain copies of the form by contacting the Office of Emergency Services and Homeland Security.
We received a DSW ID card back for one of our Department's employees, but the photo on the card does not match the name. What should we do? Return the incorrect card to your Human Resources Representative with a detailed note explaining the problem and requesting a corrected card. Your Human Resources Representative will need to query your PeopleSoft record to confirm that the photo and employee record are mismatched. If the employee record and photo are accurate, a new card should be requested from the Department of Human Resources. The new ID card will be produced and distributed at the monthly Citywide Human Resources Group Meeting. If you have not had your photo taken, you will need to do so at the Department of Human Resources before a corrected card can be issued. A corrected card will be issued to you after your photo is taken.
We have several employees who haven't been able to get their photos taken yet because of vacations, leaves, or other scheduling issues. What should we do? The department Human Resources Representative should query the employee's record to confirm that no photo exists on the Employee Photo panel in PeopleSoft. Employees who have not had their photo taken will need to come to the Department of Human Resources for a photo. Drop-in hours for photos are Monday through Friday 10 a.m. to 4 p.m. Once the employee's photo has been taken, an ID will be issued to the employee the same day. If a large number of employees (20+) in your department require photos, please contact the department of human resourcesto arrangean onsite photo session, at your department.
I am a new employee. How do I get my DSW ID card? Once your department has entered your personal information into PeopleSoft, you will need to come to the Department of Human Resources to have your photo taken. It is very important that your department enter your information into PeopleSoft, otherwise we will not be able to produce an ID for you.
I changed departments and they took my ID card back when I when I left. How do I get a new ID? As part of your appointment processing a new ID card should have been ordered. The new ID card will have your current department assignment on the back and will be printed once your new department has updated your personnel record in PeopleSoft. Your new ID card will be distributed to your Human Resources Representative at the monthly Citywide Human Resources Group Meeting.
I changed divisions, but remained at the same department. The division on the back of my card isn't correct. How do I get a new ID? Going forward, new DSW ID cards will not be printed with divisions on them. In some cases the division or address listed on the back of existing cards is not accurate or may be the location of the department's human resources offices. This is a result of some information not being accurate in the PeopleSoft system. Because the employee's department has not changed, new cards are not being issued for changes in division.
I am leaving my job with the City/I am transferring to a new department. What do I do with my old DSW ID card? Human Resources Representatives are responsible for collecting all keys, ID Cards, access codes, or other materials issued to employees granting access to department systems, procedures, offices etc. Once the ID Card has been collected, it should be destroyed and disposed of appropriately.
I lost my DSW ID Card. How do I request a replacement ID from? You will need to contact your department Human Resources Representative. Your Human Resources Representative will then request a replacement ID card from the Department of Human Resources. DHR will produce a replacement card for you. Your Human Resources Representative will sign for the card at the monthly Citywide Human Resources Group Meeting and will distribute the card to you. All requests for replacement cards must be routed through your Human Resources Representative. In emergency cases, and only after contacting the Department of Human Resources, your Human Resources Representative or you, may pick up a replacement card at 44 Gough during drop-in hours: Monday through Friday 10 a.m. to 4 p.m.
I don’t like my photo. Can I get a new photo taken and have my ID reissued? Your personnel officer may request that a new photo be taken if, in their determination, the photo no longer accurately provides identification of the employee. These cards have a cost and in most cases will not be reprinted for this reason. The purpose of the ID is to ensure that in the event of a disaster you have the proper credentials to be quickly identified as a City and County of San Francisco employee. Additionally the ID may also be used to track time and location of personnel.
